There will be an accountable care planning system based on an on – going individual assessment of strengths and needs with specific emphasis on Haven Home Care (UK) Limited’s Needs Assessment through a Service Delivery plan.
To meet the requirements of the Domiciliary Care Standards and Regulations, Service Principles and other relevant legislation.
